- If you take medications, bring a list if you cannot
remember all of them
- Please complete and bring your Patient Information Form
- Please bring both your dental and medical insurance
cards with you
- If you will be sedated under general anesthesia
(commonly referred to as going to sleep for the procedure):

DO NOT eat or drink ANYTHING past midnight the night prior to your
appointment. (no water, coffee, or gum)
- DO NOT smoke or use chewing tobacco the day of your procedure
- A responsible adult MUST accompany you, take you home, and stay with you for at least 6 hours after surgery (you cannot walk, drive, take a bus, or take a taxi home by yourself).
