We are pleased to have you visit our clinic and we would like to provide you some introductory information to make your visit with us more relaxing and efficient.

Our clinic opened in late May of 2002, but Dr. Ed May and our staff have been serving patients in Fargo, Moorhead and the greater Red River Valley area for several years.  Please follow the links to our staff pages if you would like to learn more about Dr. Ed May or About Our Staff.

Many patients have procedures performed with a local anesthetic, and, if this is true for you, no additional preparations are necessary before your surgery unless you are otherwise instructed by your dentist or nurse.

To reduce the amount of paperwork when you arrive for your visit, please complete our  Patient Registration Form before arriving for your appointment and bring it with you.  Whenever possible, we mail you this form along with a detailed estimate so you can make appropriate financial arrangements before your visit. 

Patient balances are due on the day you receive services.  We do not finance patient balances beyond the date of service, but make financial services available to you.  Please see our Financing Options to review our financial policy.

Protecting the privacy of your personal information is very important to our clinic.  We have established policies and procedures to protect your privacy, in accordance with state and federal laws.  Please read our Notice of Privacy Practices to acquaint yourself with how we will use and protect your personal information.
